Patient Portal - Patient Forms: If you are unable to use the Patient Portal, please print and complete the new patient forms ahead of time. You can find these forms here.
- Government-issued photo ID: Please bring a valid identification card such as a driver's license, passport, or any other government-issued photo ID. This helps us verify your identity accurately.
- Insurance card: If you have health insurance coverage, kindly bring your insurance card along. This will enable us to process your insurance claims and ensure that you receive the maximum benefits available to you.
- Current list of medications: It is crucial for our healthcare team to have a comprehensive understanding of your current medications, including prescription drugs, over-the-counter medications, and any supplements you may be taking.
